What To Expect From Physical Therapy
For First-Time Patients
Print and fill out the following forms and bring them to your first visit: Medical History Form, Financial Agreement Form, Medical Records/HIPPA Disclosure Form, and Patient Information Form.
You will have the opportunity to read the “Notice of Privacy Practices” at Restore on your initial visit. Please note that Restore knows you hate paperwork. However, this information is used to show your health insurance provider your necessity for “skilled care.”
Your Initial Physical Therapy Visit
Please arrive 15 minutes early to complete a current functional outcome measure. We will obtain baseline measurements of range of motion, strength, and arm girth (to detect swelling). You will be instructed on simple exercises based on your phase of recovery. Your home exercise program and patient education begin here. This time is for you! We will go at your pace. Please dress comfortably.
Your Follow-Up Visits
Depending upon where you are in your recovery, a physical therapy program to suit your needs will be developed. In addition to manual treatment to restore range of motion, reduce swelling or lymphedema, and promote scar tissue mobility, your physical therapy program will consist of slow, progressive strengthening and cardiovascular exercises to address the side effects and long-term issues associated with cancer, chemotherapy, and radiation.
What You Can Expect Upon Completion of Physical Therapy
It will be important to maintain the goals you have achieved and continue to lead a healthy lifestyle. Following clearance from your physician, we suggest transitioning to a wellness program under the guidance of a trained professional.
Our Vision & Story
Paige Sims started Restore Physical Therapy with a vision to do things differently. She saw the need for an oncology rehabilitation clinic because of how many individuals were surviving cancer, but their restoration and recovery journey was an afterthought, being overlooked and/or not a priority for these survivors’ who encompass the word “life-givers, life savers,” but they were being left behind to pick up the shattered pieces essentially limping along scared and alone. Dr. Julie Silver explained it this way, “If a patient had a stroke, you would never tell them, ‘this is the new normal if you want to get better, figure it out on your own.”
I have been a Physical Therapist for 20 years, practicing off and on, primarily in the acute care/hospital setting and orthopedic/geriatric outpatient setting. My heart has been set on fire with a passion for helping people who are recovering from the emotional and physical trauma that has kept them from moving and living to their fullest potential. I want to be a lifeline for my patients in the arena of rehab to help them move into hope, healing, and restoration. I recently heard my pastor’s wife, Amy Groeschel, say, “There is WEIGHT in the wait, ladies.” This is so true! You get to decide what you are doing for your body, mind, soul, etc. during this time of recovering- what are you doing while you are waiting for this season to pass, while you are waiting for the next phase, and while you are possibly in that season of growing stronger and more confident and more sure than ever before of what you want your life to be like.
I know you get to choose who is on this journey with you, and I know cancer might be a small part of your journey, but it is not your finish line. You get to choose what your best “yeses” are right now, and I would love to help you reach your goals. We are not meant to DO life alone. We need encouragers, faith-builders, cheerleaders, coaches, experts, motivators, friends, and family. It doesn’t make us weak to need others. We all help make each other stronger!
